I hereby grant permission to the Garrett County Arts Council (GCAC) to transport my artwork to and from Grant's Mercantile and The Gallery Shop from May 2025 to January 2026.

By signing this consent release form, I understand and agree to the following terms and conditions:

I acknowledge that GCAC will take reasonable precautions to ensure the safety and security of my artwork during transportation. However, I understand that there are inherent risks involved in transporting artwork, including but not limited to damage, theft, or loss. Therefore, I understand that all damage is subject to the insurance policy of GCAC, and I will be compensated for damage in accordance with GCAC policy.
